MRI diagnostic markers in Cushing´s disease

Project goals

Strategy and clinical results of ACTH pituitary adenoma resection are influenced by preoperative magnetic resonance imaging (MRI).Conventional MRI, however, is not sufficiently sensitive and specific to determine the exact location of the tumor. Relapse of hypercortisolism is frequent. The aim of the project is to assess the sensitivity and specificity of a new MRI protocol containing sequences and SPGRE FSPGRE conventional and optimized sequence to determine the localization of pituitary adenoma in patients with Cushing's disease. Data acquired preoperatively will be correlated with the preoperative MRI. The sensitivity and specificity of the new sequences will be assessed. The study will include patients with ACTH pituitary adenoma, planned for neurosurgical resection, cca 25 sujects in total. The project will validate miniinvasive and low-cost diagnostic method which can guide the strategy of surgical resection, improve the clinical outcome and reduce the procedural morbidity.

    Strategie a výsledky resekce adenomu hypofýzy secernujícího ACTH ovlivňuje předoperační zobrazení magnetickou rezonancí (MR). Konvenční MR však zatím není dostatečně sensitivní a specifická, aby stanovila přesnou lokalizaci tumoru. Recidiva hyperkortisolismu je častá. Cílem projektu je stanovit sensitivitu a specificitu nového MR protokolu obsahující sekvence SPGRE a FSPGRE a optimalizované konvenční sekvence pro určení lokalizace adenomu hypofýzy u pacientů s Cushingovou nemocí. Intraoperačně získaná data budou korelována s předoperační MR. Bude hodnocena sensitivita a specificita zmíněných sekvencí. Do studie budou zařazeni nemocní s adenomem hypofýzy produkujícím ACTH, podstupující neurochirurgickou resekci. Studie zahrne cca 25 pacientů. Projekt otestuje miniinvazivní a ekonomicky nenáročnou diagnostickou metodu, která může být v budoucnu vodítkem pro chirurgickou strategii léčby Cushingovy nemoci, zlepšit klinické výsledky léčby a snížit morbiditu neurochirurgického výkonu.

    Senzitivity MR for Cushing´s disease presented by papers was 76%, but specificity of exact tumour localisation is only 33%. The combination of MR sequences (SE,SPGR, fSPGR) led to increased sensitivity of up to 100%. Specificity reached 56% in our study.
