Cílem tohoto projektu je ověřit proveditelnost a bezpečnost/toxicitu eliminace komorové tachykardie (KT) související se strukturním postižením srdce neinvazivním výkonem, stereotaktickou radiochirurgickou ablací, která bude aplikována po selhání katetrizační ablace arytmie. Arytmický substrát je definován zobrazením jizevnaté tkáně a funkčně elektroanatomickým mapováním a stimulací v jednotlivých oblastech jizvy. Recipienty stereotaktické radiochirurgie budou pouze pacienti po 2 neúspěšných katetrizačních ablacích pro monormorfní KT (z nichž jedna bude provedena v expertním centru) s klinickými recidivami arytmie. Cílovým objemem pro stereotaktickou radiochirurgickou ablaci v jednorázové dávce 25 Gy bude kritická oblast substrátu definovaná na základě zobrazovacích a funkčních metod. Primárními cíli bude doba do indikace radiochirurgie po expertení reablaci, redukce celkového množství setrvalých komorových arytmií, neinducibilita během programové stimulace komor a rozvoj akutní/pozdní poradiační toxicity.

Project results evaluation
The project "STereotactic Ablative Radiosurgery of recurrent Ventricular Tachycardia in structural heart disease" is innovative. It deals with a relatively unique radiosurgical treatment using Cyberknife. The authors developed a methodology for performing CT before the planned procedure. Using a relatively unique set, they then presented the results of this method in a specific group of patients, including their complications. The publication outputs are of high quality and exceeded the proposed numbers in the application for support. The project has the potential to influence treatment practice in patients with failure of current treatment options for recurrent ventricular arrhythmias. The implementation itself is in accordance with the tender documentation. The funds are also being drawn down without any shortcomings.
