Nové diagnostické a prognostické markery v preoperační a postoperační péči o pacienty s nádory štítné žlázy

Nádory štítné žlázy jsou nejčastější endokrinní malignitou. Molekulárně genetické markery pomáhají v diagnostice a prognóze onemocnění a při stanovení rizika malignity u biopsií uzlů štítné žlázy (FNAB), kdy lze optimalizovat pre- i postoperativní management pacientů. Projekt navazuje na naši mnohaletou práci, kdy u rozsáhlých souborů dospělých i dětských pacientů systematicky identifikujeme genetické alterace a vhodné markery zavádíme do klinické praxe. Cílem projektu je rozšířit portfolio molekulárně genetických markerů, které by přispěly k diagnostice a prognóze i u pacientů, kteří nejsou nositeli majoritních genetických alterací (cca 35 % pacientů). U této skupiny dospělých pacientů chceme detekovat přítomnost fúzních genů a minoritních onkogenů a studovat korelaci s fenotypem. Plánujeme též studovat rozdíly miRNA a methylačních profilů u geneticky a klinicky různých skupin nádorů štítné žlázy a zhodnotit jejich klinický dopad. Výsledky chceme využít pro zlepšení stratifikace rizika malignity a agresivity onemocnění u FNAB vzorků, rozšíření našeho diagnostického schématu a zpřesnění prognózy pacientů.

Project results evaluation
The content-related and publication objectives of the project were achieved. Objectives 1 and 2 were fulfilled completely, while Objective 3 was met with minor reservations as noted below. I particularly appreciate the high level of publication activity of the results, which exceeded the original targets. The project resulted in a number of publications with international impact, and some of these have been cited in international guidelines concerning the diagnosis and management of thyroid tumors. The data obtained by the investigators are highly valuable from the perspective of research into thyroid oncogenesis and are promising for the search for new therapeutic and diagnostic approaches. However, in their current form, they are not sufficient to justify changes to established diagnostic procedures or the implementation of these methods into routine clinical practice.
