Anální karcinom (AK) patří mezi malignity se stoupající incidencí, hlavně ve specifických populacích mužů, kteří mají sex s muži (MSM), a HIV pozitivních pacientů. V těchto skupinách je incidence až 131 případů na 100 000 obyvatel. Vzhledem k rostoucímu počtu HIV pozitivních pacientů nejen v České Republice, zejména u MSM, je včasná diagnostika prekanceróz základním předpokladem v boji s tímto onemocněním. AK je svou etiologií a biologickým chováním velmi podobný karcinomu děložního hrdla, proto i vyšetřovací a screeningové postupy jsou téměř identické. Základem je klinické vyšetření a cytologická analýza stěru sliznice. Anální cytologie nepatří mezi vysoce senzitivní ani specifická vyšetření. Naopak metylace DNA tumor-supresorových genů se takovou metodou být zdá, s již potvrzenou efektivitou ve screeningu karcinomu děložního hrdla. Tento výzkum by měl ověřit metylaci DNA jako novou diagnostickou a potencionální screeningovou metodu pro AK a porovnat její senzitivitu a specifitu s již zavedenými vyšetřovacími metodami u imunokompetentních i imunosuprimovaných pacientů.

Project results evaluation
The project’s main benefit was the introduction of a pilot rectal cancer screening program for high-risk patients in the Czech Republic. A new algorithm for rectal cancer screening of high-risk individuals, supplemented by molecular genetic markers, was created and published based on the study’s data. Thanks to the successful implementation of the project, the rectal cancer screening program, now part of the National Screening Center and, in cooperation with the Ministry of Health, is planned to continue. The project results are also significant to other medical fields such as infectious disease, microbiology, and oncology. The presentation of the study’s results at conferences was well above average. {The study’s results have been presented at 15 conferences, which was well above expectations.} So far, three articles have been published – two in journals with IF and one in the Journal of Science Education and Technology (JOST).
