Giant cardiac lipoma - Potentially deceptive entity in differential diagnostic imaging

  • Original language description

Primary cardiac tumors are very rare. Their incidence at autopsy ranges from 0.17 to 0.19%. Cardiac lipoma presents approximately in 10–19% of the primary tumors of the heart [1]. These well-encapsulated tumors are typically composed of mature fat cells [2]. Lipomas commonly occur in middle-aged and older adults [3], affecting both genders equally [4]. Most of them are subendocardial or epicardial, and only 25% are found in the myocardium [3]. The most frequent location is the left ventricle and the right atrium, but almost any location of the heart is possible [5]. Patients with cardiac lipoma are usually asymptomatic until local mass effect leads to hemodynamic compromise and cardiac insufficiency. Symptoms commonly reported encompass fatigue, dyspnea, palpitation, syncope and even chest pain, which are frequently a result of coronary artery or cardiac conductive system involvement. A case of sudden death caused by cardiac lipoma has also been reported [6].
