Pharmacotherapy of idiopathic inflammatory myopathies (IIM) remains mainly empirical. The article provides an update of published evidence with a suggested treatment algorithm. Glucocorticoids are stillat the core of the current armamentarium. A number of other immunosuppressive and/or immunomodulatory drugs are being used based on a limited data regarding their efficacy. The most promising biological agent in the treatment of IIMs is currently rituximab
