    Partial Preservation of the Anterior Capsule in Total Hip Arthroplasty to Decrease the Risk of Iliopsoas Impingement

    Mechanical irritation of the iliopsoas muscle resulting from a prominent acetabular cup leads to a well-known complication after total hip arthroplasty, referred to as the iliopsoas impingement. Clinically, patients present with persistent postoperative groin pain triggered by active flexion of the hip. Various risk factors have been identified for the development of iliopsoas impingement, including excessive anterior cup overhang, usage of larger components, higher native acetabular anteversion, female gender, younger age, or prior spine fusion. In this technical report, we describe a simple technique that partially spares the anterior capsule in total hip arthroplasty, and aims to minimize the contact between the iliopsoas muscle and the implanted acetabular cup to decrease the risk of postoperative iliopsoas impingement.
