    A 32-year-old man, with no history of headaches or recent trauma, presented with a new-onset, left-sided, retro-orbital, stabbing pain of fluctuating intensity (pain maximum in the night) over the last 20 h. We noted ipsilateral nasal congestion, rhinorrhea, lacrimation and lid ptosis. Neurological examination revealed a left Horner's syndrome accompanied by ipsilateral conjunctival injection (Fig. 1a, b) and ipsilateral superficial temporal artery (STA) dilatation (Fig. 1c, d). We were unable to document ipsilateral anhidrosis. A first episode of cluster headache (CH) was suspected. Certain atypical features (absence of pain-free periods, variability of pain intensity during the past 20-h period, lack of prior headache history) prompted further investigation with carotid duplex (Fig. 1e) and brain-MRI (Fig. 1f) leading to the diagnosis of a spontaneous left internal carotid artery dissection (ICAD) by disclosing an intramural hematoma in distal extracranial left internal carotid arter
