Introduction: Iron deficiency (ID) is a common comorbidity in patients with advanced heart failure (HF). It is associated with decreased performance status, frequent hospitalizations for HF decompensation, and lower quality of life. There is an ongoing debate about the optimal definition of ID and the selection of patients benefiting from iron supplementation. Previously, ID in HF was defined as ferritin < 100 μg/l, or 100-299 μg/l if transferrin saturation (T-sat) was < 20%. In light of new findings, the definition of ID has been simplified in the Update ESC Guidelines for the Diagnosis and Treatment of HF to T-sat < 20% or ferritin < 100 μg/l.
