Original language description
Exposure of the most distal peroneal artery (DPA) is technically challenging due to the medial tibial flare, the need for fibular resection with the lateral approach, and the necessity for patient repositioning when proximal inflow or vein harvest is performed through a posterior approach. The anterior position of the DPA directly beneath the interosseous membrane allows exposure through the anterior compartment, thereby avoiding fibular resection and patient repositioning. The anterior trans-interosseous approach to the DPA eliminates the need for bone resection, avoids patient repositioning, and enables bypass grafting to the peroneal artery under challenging circumstances. When autologous conduits are unavailable, allografts may be used as bypass material.
