Original language description
The limitations of surgical revascularisation and pharmacological treatment in peripheral arterial occlusive disease (PAOD) are well recognized. Therapeutic options for critical leg ischemia are consequently limited to percutaneous transluminal angioplasty (PTA) or surgical revascularisation. Unfortunately, many patients with critical leg ischemia are poor candidates for either procedure. Therapeutic angiogenesis is a novel promising tool to treat these patients. Experimental and clinical and trials ofgene transfer for therapeutic angiogenesis have already shown some clinical efficacy. This review is focused on gene transfer techniques in preclinical and clinical therapeutic angiogenesis, angiogenic growth factors, vectors, delivery methods and routes. The results of clinical and experimental studies, safety and side effects of gene therapy, and the perspectives of future research are also discussed.

Czech description
Omezení chirurgické a farmakologické léčby periferní arteriální okluzivní choroby (PAOD) revaskularizací jsou dobře známa. Terapeutické možnosti pro léčbu kritické ischemie dolních končetin jsou omezeny na perkutánní transluminální angioplastiku (PTA) nebo chirurgickou revaskularizaci. Bohužel, mnoho pacientů s kritickou ischémií dolních končetin není kandidáty ani pro jeden z těchto postupů. Terapeutická angiogeneze představuje nový slibný nástroj pro jejich léčbu. Experimentální a klinické zkoušky přenosu genů pro léčebnou angiogenezi již prokázaly jisté klinické výsledky. Tato práce je zaměřena na techniky přenosu genů v preklinické a klinické léčebné angiogenezi, růstové faktory angiogeneze, vektory a jejich využití. Dále jsou také diskutovány výsledky klinických a experimentálních studií, bezpečnost, vedlejší účinky genové terapie a perspektivy budoucího výzkumu.
