    Regarding Insulin Initiation in Type 2 Diabetes Mellitus and the Risk for Hypoglycemia

    We read with interest the review article by Jaqueline Smith, &quot;Practical Tips for Insulin Use in Primary Care&quot; that has been published in the Canadian Journal of Diabetes. It provides a useful outline of the basic framework for the initiation of insulin therapy in people with type 2 diabetes who have failed on oral antidiabetes therapy and how prandial and basal insulins can be used. While the risk of hypoglycemia is mentioned, it is introduced primarily as a potential barrier to the initiation of insulin therapy from the viewpoint of the patient as user and the medical practitioner as provider, with only a cursory mention of the risks associated with various insulin preparations.
