Portal hypertension as a consequence of liver cirrhosis is responsible for its most common complications: ascites, spontaneous bacterial peritonitis, hepatorenal syndrome, hepatic encephalopathy and the most important one - variceal hemorrhage. Varicealbleeding results in considerable morbidity and mortality. This review covers all areas of importance in the therapy of acute variceal hemorrhage - endoscopic and pharmacological treatment, transjugular intrahepatic portosystemic shunt, surgery and balloon tamponade. Indications and limitations of these therapeutic modalities are widely discussed.

Portální hypertenze jako důsledek cirhózy jater je odpovědná za její nejběžnější komplikace: ascites, spontánní bakteriální peritonitida, hepatorenální syndrom, jaterní encefalopatie a nejdůležitější z nich ? variceální krvácení. Tento typ krvácení je odpovědný za významnou morbiditu a úmrtnost. Tento přehledový článek se zabývá všemi oblastmi důležitými při léčbě akutního variceální krvácení ? endoskopickou a farmakologickou intervencí, transjugulárním intrahepatickým portosystemickým odklonem, chirurgickým zákrokem a balónkovou tamponádou. V rámci prezentace jsou také široce diskutovány indikace a omezení těchto léčebných přístupů.
