This study evaluates the complete clinical and hemodynamic response (CCHR) in patients with obstructive hypertrophic cardiomyopathy (oHCM) treated with alcohol septal ablation (ASA). The analysis included 457 symptomatic patients treated between 1997 and 2023, with a mean follow-up of 7 years. The primary endpoint (CCHR), defined as a residual LVOT gradient <30 mmHg, NYHA class I/II, and absence of major complications, was achieved in 53.4% of patients. Key predictors of not achieving CCHR were female sex and higher baseline interventricular septal thickness. The findings highlight the efficacy of ASA in improving long-term outcomes for severely symptomatic oHCM patients.
