The cerebellum's role in motor laterality (handedness and footedness) remains unclear. It is a lateralised organ, and one functional manifestation of this lateralisation might be neocerebellar extinction syndrome, clinically identifiable as minor muscle hypotonia and moderate passivity in the cross-preferred. We studied how physiological neocerebellar extinction syndrome relates to handedness and footedness, measuring damping coefficient and frequency differences during controlled falls via 3D kinematic analysis. Data were collected from the dominant and non-dominant upper and lower extremities of 40 left- and 40 right-side dominant adults (mean age = 24.7 +- 0.92 years). Using a linear mixed model, we found that hand dominance significantly increased hand damping, with the dominant hand showing higher coefficients than the non-dominant hand, regardless of handedness. Footedness and foot dominance had little effect on leg damping individually or combined. The combined impact of right-handedness and dominance on hand frequency was minimal. Similarly, footedness and foot dominance individually didn't affect leg frequency. However, right-footed, right-dominant individuals exhibited significantly higher leg frequency. Our findings suggest that neocerebellar extinction syndrome is more pronounced in the upper extremities, likely due to different spinocerebellar pathways. Additionally, limb segment characteristics are more significant than neocerebellar extinction syndrome in determining limb oscillation frequency.
