Aim: Diabetic foot syndrome associated with atherosclerotic disease of the crural arteries often results in chronic critical leg ischemia, with gangrene at the terminal stage. Pedal bypass surgery is often the only method of limb salvage. Methods: In 2001 to 2003, we carried out pedal bypass in 30 patients. Results: At 36 months of folow-up, the cumulative primary and secondary graft patency were 87 % and 90 % respectively; the cumulative limb salvage rate was 93 % (Kaplan Meier analysis).

Czech description

Úvod: Kombinace diabetické angiopatie a aterosklerotického postižení bércových tepen často vede k rozvoji chronické kritické ischemie dolní končetiny, syndromu diabetické nohy a v poslední fázi ke vzniku diabetické gangrény. Revaskularizace pedálním bypassem je často jedinou možností jak zachránit končetinu. Metodika: V letech 2001-2003 jsme implantovali pedální bypass 30 pacientům. Výsledky: V 36 měsíčním sledování byla kumulativní primární a sekundární průchodnost bypassů 87 % a 90 %, kumulativní pravděpodobnost záchrany končetiny byla 93 % (Kaplan - Meier analýza).
