3D modeling for mandibular reconstructions: Case report.

  • Original language description

Oral Squamous cell carcinoma (SCC) is the most common cancer of the lip and oral cavity (comprises nearly 30 % of all oral cavity cancers) and it usually occurs in patients over the age of 50 years. The major risk factor of SCC occurrence is the exposure to UV-radiation next to smoking, in the etiopathogenesis of SCC. Treatment of the lip tumours mainly consist of surgery or radiotherapy. In advanced stages of disease the surgery is often combined with subsequent radiotherapy, eventually in combination with chemotherapy, i.e. systemic treatment. In advanced stages of the disease, surgical tumour resection becomes more challenging, thus raising specific considerations for reconstruction relative to the extent of resection. The present case describes the surgical management of a 69-years-old woman with recurrent carcinoma of the lower lip. She underwent repeated resections in years 2007, 2008, and in January 2018. Since July 2018, there occurred an increasing perimandibular resistance on the right side in close contact with mandible. According CT, bone usurance was not proven. Tumour resection with partial segmental mandibulectomy and block neck dissection were indicated. In this report, we present the surgical approach we used to replace the resected portion of the mandible using 3D modelling and the use of a freely transferred vascularized bone graft.
