The paper focuses on the use of diffusion tensor imaging (DTI) in the evaluation of one case of Chiari III malformation. In the case discussed, DTI was used to delineate the position of large descending tracts within the malformation and the reconstructed images were used to plan the surgical procedure. The clinical and imaging findings, the technical aspects of the DTI fiber tract reconstruction and the outcome are sumarized.
