    The abstract presented at the International Stroke Conference 2025 (Los Angeles, USA; 5-7 February 2025) examined pre-hospital delay for acute ischemic stroke using the international Registry of Stroke Care Quality (RES-Q) for 2022-2023. Median onset-to-door time (ODT) was analyzed across 25 countries and compared by mode of arrival (emergency medical services vs. non-EMS), after excluding secondary transfers and records with missing/erroneous data. Among 155,532 patients, median ODT was shorter for EMS arrivals (193 min) than for non-EMS arrivals (309 min). The proportion of EMS arrivals varied substantially by region (highest in Europe) and EMS transport was associated with a higher likelihood of receiving intravenous thrombolysis. The authors conclude that strengthening EMS infrastructure may reduce arrival times and improve access to acute reperfusion therapy globally.
