This study evaluated the benefit of early initiation of high‑efficacy disease‑modifying therapies (HE‑DMT) in patients with relapsing–remitting multiple sclerosis using data from the Australian MSBase cohort and the Czech national ReMuS registry. Patients initiating their first DMT between 2013–2018 with at least five years of follow‑up were included. After balancing the cohorts using propensity score weighting, early HE‑DMT initiation was associated with a lower risk of relapses, 6‑month confirmed disability worsening, and reaching EDSS 3, alongside a higher probability of disability improvement. Subsequent Bayesian survival modelling incorporating results from prior international registry comparisons confirmed and further refined these findings. The study provides independent and internationally consistent evidence supporting early intensive treatment as an effective strategy in multiple sclerosis management.
