The contribution is devoted to the description and the application of the mathematical model of groundwater flow and transport based on the mixed-hybrid formulation of the FEM. The model was applied on a problem of identification and evaluation of the transfer of contaminants between two aquifers divided by an aquitard in the area of Stráž pod Ralskem, Czech Republic.
