Lower extremity length discrepancy (LELD) and angular deformities in the knee region belong to frequent problems that are solved by paediatric orthopaedic surgeon. The role of LELD, both a biomechanical impediment and a predisposing factor for associated musculoskeletal disorders has been a source of controversy for some time. Compensatory mechanisms during walking are complex and result in kinematic alterations in the lower extremities and pelvis - biomechanically significant shortening represent so-called pre-osteoarthritis [4, 5]. Mild angular deformities represent normal variations of lower limbs development. Nevertheless, bowlegs after 2 years of age are consider as abnormal and knock knees after 7 years of age need more evaluation to exclude pathologic causes. Severe angular deformities of lower extremities are not only a cosmetic problem. Without treatment they soon lead to pre-osteoarthritis and osteoarthritis of big joints of legs in young adults. Nowadays progress in surgery and auxology enables the treatment both around knee deformities and LELD by mini-invasive surgery. Epiphysiodesis is paediatric surgical procedure used to close a growth plate at the upper and/or lower extremities. The aim of this surgery is stop an abnormal growth on one side of the physis and correct an angulation in the level of growth plate which is the most appropriate biomechanical place for deformity correction.
