In this chapter, the authors contend that health and environmental damages are tightly linked. What is not that clear yet is which comes first: does health and environmental damage (quality) rise (fall) with income or, conversely, does income rise (fall) prevent (foster) both? In this chapter, the authors show that the trade-off between health and wealth depends strongly on what is happening elsewhere and that the wealthier and healthier is not necessarily the cleaner.
