    Posterior Malleolar Fractures Changing Concepts and Recent Developments

    Injuries to the posterior malleolus (PM) have a prognostic impact for patients with ankle fracture-dislocations. The three-dimensional outline of the fragments as seen on CT, involvement of the fibular notch of the distal tibia (incisura), and the presence of depressed intercalary joint fragments seem to be of greater therapeutic relevance than the size of the fragment and the amount of the fractured articular surface alone. Although extraincisural fractures (type 1 according to Bartoníček and Rammelt) can be treated nonoperatively, operative treatment of type 2 to 4 fractures aims at reconstruction of the posterior tibial plafond, the fibular notch, and the integrity of the posterior tibiofibular syndesmosis. The individual choice of approaches depends on the fracture pattern of the PM and associated injuries to the medial and lateral malleolus.Direct open reduction and fixation of PM fragments via posterolateral and posteromedial approaches is biomechanically more stable than indirect reduction and anteroposterior screw fixation.
