The retrospektiv analysis of 125 consecutie patients with hemispheral stroke (78 male, mean age 66,0 +/- 12,1 years) was performed. Clinical-difussion mismatch (CDM) was defined as NIHSS >/= 8 and DWI volume > 25 ml. Volume of infarction was quantified at admission and 24 hours later. Neurological deficit was evaluated using NIHSS at admission and 24 hours later. Clinical outcome at day 90 was evaluated using modified Rankin Scale (mRS). CDM was detected in 61 (48,8 %) patients and it was missingin 31 (26,4 %) cases. Patients without CDM had significant progression of infarction (p < 0,0001) after 24 hours. Patients with CDM had better clinical outcome.
