Narcolepsy type 1 (NT1) is a rare disease strongly associatedwith hypocretin/orexin (hcrt) deficiency and HLA DQB1*0602 positivity.The pathophysiology is linked to the damage of hcrt neurons in hypo-thalamus probably by an auto-immune process. The diagnosis is basedmainly on clinical history and polysomnographicfindings, including theMSLT test. The PSG/MSLT test fails to prove the diagnosis in some patients.Finding of the low hcrt level in CSF supports the diagnosis of NT1. Based onsensitivity/specificity analysis, the optimal cut-off value of hcrt in CSF forNT1 was set to 110 pg/ml. The laboratory in Ostrava, Czech Republic ex-tends a spectrum of laboratories providing this measurement over theentire world
