Introduction: VWF and f VIII are determinants of thrombi creation. ADAMTS13 influence thrombus dissolution. Objectives: Higher levels of Von Willebrand Factor (vWF), factor VIII (f VIII) and low Von Willebrand factor-cleaving protease (ADAMTS13) enzyme are factors that may influence the outcome in stroke patients treated with mechanical thrombectomy. Methods: 50 patients (27 males, mean age 68 ± 11.0 years) with acute ischemic stroke and occlusion of middle cerebral artery (MCA), internal carotid artery (ICA) or basilar artery (BA) treated with mechanical thrombectomy were enrolled to the study between August and December 2016. Baseline blood levels of VWF, fVIII, ADAMTS13 were correlated with procedure length, recanalization success, National Institutes of Health Stroke Scale (NIHSS) at baseline and day 7, and 3-month outcome in modified Rankin Scale (mRS). Results: Significant correlations were found between higher serum levels of vWF and fVIII versus low levels of ADAMTS13 in patients with severe strokes (baseline NIHSS > 15) and in patients with worse clinical outcome (NIHSS ≥ 10 at day 7 and mRS ≥ 3 at day 90). No significant correlations have been found between factors' levels and procedure duration or recanalization success. Conclusion: Levels of platelets adhesion factors VWF, fVIII and ADAMTS13 are in correlation with stroke severity and stroke patients' outcome after thrombectomy.
