In a cohort of 169 patients with multiple myeloma, treated at our department with bortezomib-based chemotherapy, we assessed the incidence of herpes zoster reactivation in patients with and without low dose acyclovir (200mg once daily) prophylaxis. In 78patients ( 46%) without any prophylaxis, herpes zoster occured in 21 cases (27%). In 92 patients (54%) with low-dose acyclovir prophylaxis there was only one case of herpes zoster in a patient with skipped medication. Most patients with herpes zoster underwent the reactivation within the first three cycles of the treatment. Low-dose acyclovir in multiple myeloma patients treated with bortezomib creates a sufficient prophylaxis of herpes zoster reactivation.
