Original language description
Chlumsky solution containing camphor, phenol and ethanol is used in human and veterinary medicine for its disinfecting effects. In the Czech Republic, fatal intoxications with disinfectants are quite unusual. In our Department we encountered the first case of fatal intoxication by Chlumsky solution in a 39-year-old man. Complete toxicological analysis was performed with the biological material collected at the autopsy. Blood and urine were examined by GC-FID method for the presence of alcohol, urine was also examined by an indicative immunochemical test for the presence of addictive substances. The stomach content, intestinal content, liver and kidney were analyzed by thin-layer chromatography (TLC), gas chromatography with mass spectrometry (GC-MS) - after acid-base extraction with ether. The unknown fluid from the stomach and intestinal contents were analyzed by TLC. The serum was examined by GC-MS and liquid chromatography with mass spectrometry (LC-MS) after acid-base extraction and after simple deproteination by methanol. DRI method showed only benzodiazepines in the urine. TLC showed the presence of caffeine in all materials, in stomach and intestinal contents phenol was also found and urine contained zolpidem. GC-MS and LC-MS were used to detect the presence of phenol and camphor. Both substances were in lethal concentrations, zolpidem was in toxic concentration, drovataverine and alprazolam were in therapeutic concentrations. In addition to sophisticated equipment of toxicology laboratory with quality UPLC-MS and GC-MS systems, knowledge of circumstances that led to death is necessary for objective diagnosis of camphor and phenol intoxication. The paper presents a case of death of 39 year old man after oral ingestion of Chlumsky solution, which was combined with zolpidem, alprazolam and drotaverine. It describes the use of UPLC-MS and GC-MS in the analysis of these compounds in biological material. The work also points out possible pitfalls in the diagnosis of phenol and camphor intoxication.
