Bouveret syndrome is a rare form of gastric outlet obstruction caused by an impacted gallstone in the pylorus or proximal duodenum after its passage through a biliodigestive fistula. This case report with video presents its management using intracorporeal electrohydraulic lithotripsy furthermore followed by endobiliary radiofrequency ablation for biliary tumor ingrowth in a complex procedure totaling more than 4.5 hours
