    Multi-hit TP53 confers the poorest survival in multiple myeloma in the era of novel therapies

    This study investigates the prognostic impact of multi-hit TP53 in patients with multiple myeloma treated in the era of novel therapeutic agents. Multi-hit TP53, defined as the presence of del(17p) combined with a TP53 mutation or at least two TP53 mutations, was analyzed in a real-world cohort of 204 patients. The results demonstrate that patients harboring multi-hit TP53 exhibit the poorest progression-free survival and overall survival compared with other high-risk cytogenetic groups and standard-risk patients. Multi-hit TP53 was associated with advanced disease stage and treatment resistance, highlighting its role as a critical adverse prognostic factor. These findings support the routine assessment of TP53 abnormalities for improved risk stratification and therapeutic decision-making in multiple myeloma.
