Background: Dependence on heroin (and other illicit opioids) is a serious health and social problem. In the Czech Republic, there are estimated 4.5 thousand of problem heroin users and 7.1 thousand of buprenorphine users (especially Subutex®) obtained on the black market. Users of heroin and other opioids are the 2nd largest drug treatment group. Treatment of opioid dependence is based on psychosocial interventions and/or pharmacological interventions (substitution treatment). Methods: Systematic review. A search of foreign and Czech scientific literature focused on the treatment of heroin addiction (or other illegal opiates) and on the factors influencing its results in specialized databases (ScienceDirect, Scopus, Cochrane Database) and other sources (Medvik) was carried out. These were texts that were published between 2000 and 2017. Conclusions: The basic predictors that have a positive effect on the success of treatment of opiate addiction are: the length of treatment, some sociodemographic characteristics (especially the age at treatment entry), some psychological characteristics. The risk factor is psychiatric comorbidity. The pressure of client´s conflicts with law increases the retention and overall success of the treatment. A positive predictor is also a family situation without major conflicts and the absence of dependence in family members and partners. Higher retention of clients showed substitution treatment than only psychosocial therapy. The success of substitution treatment is associated with higher doses (whether methadone or buprenorphine - as the most common substitution medicines), psychosocial support in the treatment but without excessive pressure to change the lifestyle of the client, and good relationships with treating staff. Taking other drugs during treatment leads to worse results. Users of opioids other than heroin and non-injecting users achieve better treatment outcomes.
