    Seven years old grey Arabian gelding was referred to the Equine Clinic in University of the Veterinary and Pharmaceutical Sciences in Brno with bilateral swelling of parotideal regions that developed after episode of fever and lethargy one year ago. Clinical examination revealed large bilateral firm and nonpainfull swelling on both parotideal regions which was bigger on the left side. Endoscopic examination was done and complete paralysis of left arytenoid cartilage was found. Left guttural pouch was filled with ingesta and right guttural pouch was filled with pus and completely deviated by the left one. Esophagoscopy, radiography and contrast radiography of guttural pouches and esophagus were done and wide communication between dorsal esophageal walland left guttural pouch was found. Its opening (5cm in diameter) was located in esophagus in the distance of 20cm caudally from cranial esophageal sphincter.
