Malignant cells in multiple myeloma (MM) produce a monoclonal immunoglobulin which is tumor-specific and can be used for the induction of specific T cells. The idiotype (Id) is expresse dat the cell surfaře of malignant plasma cells and allow the recognition and targeting of these cells by Id-specific T lymphocytes. A phase II clinical study has started in our center, investigating the efficacy and toxicity of Id conjugated with keyhole limpet hemocyanin (KLH) given as a vaccine to patiens with MM. Theaim of this therapy was to induce specific immune response directed against the tumor cells. Twelve patiens with stable disease or with slow progression not requiring systemic therapy were immunized six times with or without interleukin-2.

Czech description

Buňky mnohočetného myelomu produkují nádorově specifický monoklonální imunoglobulin, který lze použít pro indukci specifických T buněk. Idiotyp (Id) je exprimován na povrchu maligních buněk a může napomoci rozpoznání a cílení těchto buněk Id-specifickýmiT lymfocyty. V našem centru byla zahájena klická studie II. fáze, která sleduje účinnost a cytotoxicitu konjugátu Id-keyhole limpet hemocyanin (KLH) podávaného jako vakcína u myelomových pacientů. Cílem této terapie je navození specifické imunitní odpovědi cílené proti nádorovým buňkám. 12 pacientů se stabilním onemocněním příp. pomalou progesí, která nevyžadovala systematickou terapii, bylo 6x vakcinováno s nebo bez podpory interleukinu 2.
