Systemic approach to an airport incident investigation
Original language description
The paper describes the practical use of the system approach to the incident investigation at the international airport and why the airport focuses on new methods in safety engineering. The approach is shown on the test investigation of aircraft incident using the CAST methodology based on the STAMP model describing investigation steps and overall context. The outcome is compared with the traditional approach to investigation, and causal factors and safety recommendations are determined. A systemic approach significantly influences these recommendations positively. Traditional methods are proven over the years, but it is beneficial to complement them with a new approach to get the best possible lessons learned and the most effective recommendations and safety actions.
