    There is a current general tendency to shorten the period of institutional care, i.e. hospitalisation of patients, for the period necessary and then use the services of community care. Objective : is to provide the patients / clients with sufficient carein their own social environment. The paper is rather a discussion one, presenting partial results of a survey. Its main objective was to determine whether there is continuity of nursing care after the patient is discharged from the hospital. Methods: Toachieve this objective we used the method of a questionnaire survey among clients discharged from the hospital, nurses working at the hospital and nurses caring for clients in their homes. Results and conclusion: The survey showed that there are certainlimits in provision of care for discharged patients. We believe that this lack in care provision could be eliminated by following a standardised procedure.
