Brachytherapy is an important modality for cancer treatment with radiation. Dose distribution around radioactive sources is not as easily measurable as in case of external beams due to the large dose gradients and a shortage of convenient detectors, therefore QA is challenging. This contribution describes design of the audit in the Czech Republic for brachytherapy systems and shows the results obtained in 1996 – 2018 for 30 audited brachytherapy sources. Following parameters are evaluated within the audit: 1) Accuracy of TPS calculation. Monte Carlo data from TG-43 have been used for analytical model used as an independent tool to verify accuracy of TPS calculation. 2) Accuracy of source calibration. Air-kerma strength is measured by in-air method. 3) Accuracy and precision of source positioning. 4) Accuracy and precision of reconstruction process. Systematic differences in calibration techniques (well-type chamber versus in-air method with Farmer chamber) will be discussed. The most important findings will be reported to point out the weakest points in source positioning as a lessons learned. The progress in imaging methods used for brachytherapy planning is well demonstrated. Analysis of results for 30 brachytherapy audits have been performed. Based on the results, the weakest points in QA of HDR sources for brachytherapy observed in last 22 years will be reported.
