Long-term results after Senning operation for transposition of great arteries
Project goals
A prospective study on long-term results of Senning operation for transposition of great arteries in 225 survivors operated between 1984 and 1997. High risk patients with systemic ventricular dysfunction and dysrrhythmias will be identified by means of detailed echocardiographic and doppler examination, exercise test and 24 hour ECG Holter monitoring In suitable candidates with a significant right ventricular dysfunction and/or tricuspid valve regurgitation the left ventricle retraining and conversion to anatomic correction will be considered Patients with significant atrioventricular and interventricular conduction disturbances will undergo detailed electrophysiology testing to optimalize ventricular function by resynchronization pacing methods

Prospektivní studie výsledků Senningovy operace provedené v letech 1984 až 1998 u 225 dlouhodobě sledovaných pacientů s transpozicí velkých arterií. Metoda:dotazník kvality života, echokardiografické vyšetření, 24 hodinová monitorace EKG a zátěžové vyšetření. U rizikových pacientů srdeční katetrizace s testováním funkce levé srdeční komory a elektrofyziologická studie. U vhodných kandidátů s dysfunkcí pravé srdeční komory nebo s regurgitací trikuspidální chlopně bude zvážena indikace ke konverzi na anatomickou korekci. U pacientů s dysfunkcí pravé srdeční komory a s poruchami síňokomorového nebo nitrokomorového vedení bude indikována optimalizace srdeční funkce pomocí resynchronizačních stimulačních metod.

Long term results of 168 pts after Senning procedure for transposition of the great arteries are good. 15 yrs survival probability was 91%. Good tricuspide valve function was found in 72%, sinus node dysfunction in 37% and severe dysrhythmias in 8% pts.
