This letter responds to a comment by Di Vincenzo and colleagues on our study that evaluated the role of growth differentiation factor (GDF)-15 levels in the development of congestive anorexia and weight loss, as well as its association with adverse clinical outcomes in patients with advanced heart failure (HF) and reduced ejection fraction. Our study demonstrated that elevated GDF-15 levels were associated with markers of venous congestion and right ventricular dysfunction. Although we addressed the hypothesis that GDF-15 might function as an energy sensor reflecting metabolic changes in patients with advanced HF, our data do not support this theory. We did not find statistically significant differences in GDF-15 levels between patients with and without metabolic syndrome. We also discussed the possible influence of antidiabetic medications, although our ability to draw conclusions about their influence was limited by the use of a historical cohort. Based on our results, we hypothesize that GDF-15 may share common mechanisms across different clinical conditions, with right ventricular dysfunction and splanchnic congestion being the most likely factors in advanced HF. Our study suggests that hepatic congestion could be a primary source of GDF-15 in this context, although the mechanisms linking hemodynamic congestion to elevated GDF-15 remain incompletely understood.
