    To compare development of hydronephrosis, differential renal function, grow up of affected and contralateral not-affected kidney and changes of TGF beta 1 in serum and urine in a group of conservatively and surgicaly treated children with unilateral high-grade hydronephrosis. Surgery is an effective alternative for children with high-grade hydronephrosis. Decrease of differential renal function is completely reversible after additional surgery. A conservative approach brought significant decrease of hydronephrosis in additional surgery. A conservative approach brought significant decrease of hydronephrosis in only 18,5% of cases. Resolution of the obstrtuction leaded to significant decrease of TGF beta 1 in serum.
