    Beta-blockers as antiarrhythmics in septic shock: a light at the end of the tunnel?

    The attractive concept of alleviating a non-compensatory tachycardia using beta-blockers in patients with septic shock has been recently tested by two multicentric trials which either did not show a positive impact on the outcome [1] or demonstrated a possible medication associated harm (Stress-L trial). The Landi-SEP [1] randomized into each arm about 95 non-selected patients with septic shock and tachycardia, of whom one-fourth had atrial fibrillation (AF). The results of Landi-SEP do not support a stringent heart rate reduction below 95 bpm in a septic shock population with a persistent tachycardia. Similarly to the J-Land3S trial [2], the landiolol was effective in reducing heart rate and treating supraventricular arrhythmias, which are known to worsen the outcomes of the critically ill.
