    Tailored immunosuppression in biopsy-proven immune-mediated myocarditis

    This editorial discusses the study by Caforio et al., which investigates the long-term efficacy and safety of tailored immunosuppressive (IS) therapy in patients with immune-mediated, biopsy-proven myocarditis. Myocarditis is a heterogeneous inflammatory disease of the myocardium with varied etiologies and clinical presentations. While immunosuppressive therapy has demonstrated potential benefits in autoimmune forms of myocarditis, its role remains controversial, particularly in virus-negative patients. Caforio et al. conducted a propensity-weighted study including 91 patients receiving tailored IS treatment and 267 patients on conventional therapy. Despite a higher-risk profile, the IS-treated group showed similar transplant-free survival rates and biventricular function compared to controls, although with a higher relapse rate. The study underscores the importance of individualized immunosuppressive regimens, guided by myocarditis subtype, disease severity, patient comorbidities, and multidisciplinary collaboration. It highlights the need for further randomized controlled trials to refine patient selection and treatment strategies for immunosuppression in myocarditis.
