The authors discuss the clinical usefulness of individualized PET SPM analysis in the detection of pathophysiological correlates of psychopathology and in localizing the target for rTMS therapy. The article presents case studies about patients with musical hallucinations and OCD, organic schizoform disorder in temporal lobe epilepsy and depersonalization and derealization syndrome. Individualized PET SPM analysis could be a useful tool in psychiatry but it is not available for general psychiatric practice. LORETA, which uses an EEG machine is inexpensive, generally available and our preliminary clinical experience suggest that it could be sensitive to changes following rTMS and can show at least partially similar abnormalities as revealed in 18FDG PET.

Autoři diskutují o užitečnosti individualizované PET SPM analýzy v detekci patofyziologických korelátů psychopatologie a v cílení rTMS. Ve článku jsou představeny kasuistické studie pacientů s muzikálními halucinacemi a OCD, organickou schizoformní poruchou při temporální epilepsii a syndromem depersonalisalizace a derealizace. Individualizovaná PET SPM analýza může být užitečným nástrojem v psychiatrii, i když její finanční a fyzická dostupnost znesnadňuje využití v široké psychiatrické praxi. Dosavadní klinické zkušenosti s LORETA, která využívá klasických EEG přístrojů, naznačují, že by tato metoda mohla být do určité míry levnější a široce dostupnou alternativou k drahým neurozobrazovacím vyšetřením.
