    Aim of this study was to evaluate miRNa expression profiles of patients with locally advanced rectal tumours in correlation with tumour regression grade as the new predictive biomarkers of response to chemoradiotherapy. : Patients with rectal cancer who underwent neoadjuvant chemoradiotherapy followed by surgical treatment were included. Blood plasma samples and tumour tissue samples were collected before the neoadjuvant chemoradiotherapy. Expression profiles of miRNA in plasma and tissue were gained by new generation sequencing. By comparing the miRNA expression profiles prior to the treatment, and after dividing the patients into good response groups (TRG 3 + 4) and poor or no response to treatment (TRG 0 + 1 + 2), 52 miRNAs in plasma and 91 miRNAs in tissue with significantly deregulated expression (P &lt; 0.05) were identified by statistical analysis. A total of 21 miRNAs showed increased expression, and 31 miRNA showed reduced in blood plasma samples. A total of 31 showed overexpression and 60 reduced in tissue biopsy samples. Our findings suggest that circulating and tissue miRNAs could serve as potential predictive biomarkers of the response to neoadjuvant chemo-radiotherapy in rectal cancer.
