    Aim of this study was to evaluate miRNa expression profiles of patients with rectal tumors after neoadjuvant chemo-radiotherapy in correlation with final nodal positivity and T stage as potential predictive biomarkers helping in selection of patients with good response to local excision or radical resection. Blood plasma samples of patients with rectal cancer were collected at the time of restaging. Expression profiles of miRNA were gained by new generation sequencing. By comparing the miRNA expression profiles divided into groups with metastatic nodal involvement (pN +) and non-nodal (pN-) patients, 65 miRNAs with significantly deregulated expression (P &lt; 0.05) were identified. A total of 37 with increased and 28 with decreased in pN + patient samples versus pN- patient samples. 58 miRNAs with deregulated expression (P &lt; 0.05), 37 miRNAs with increased expression and 21 miRNAs with reduced expression in pT3 + 4 patient samples as opposed to pT0 + 1 + 2 patients were found in the analysis of the plasma. Our findings suggest that circulating miRNAs could serve as potential predictive biomarkers of final nodal involvement and downsizing after neoadjuvant CHRT in rectal cancer.
