Background Young adults (YA) with GI cancers represent a biologically distinct subgroup. This study examined differences in genomic profiles and indications for genomically driven therapy (GDT) between YA and average-onset (AO) patients across GI cancer types. Methods Data from 625 advanced GI cancer patients enrolled in the Czech national precision medicine platform GENESIS were analyzed. Patients were stratified as YA if aged 20–49 years(y), or AO (≥50 y). A molecular tumor board made GDT recommendations based on NGS comprehensive genomic profiling. Genomic alterations (GA), clinically relevant variants (CRV), and GDT implementation were compared.
