    Parathyroid hormone (PTH, parathormone) is a basic regulating hormone of calcium phosphate metabolism. The clinical significance of PTH assessment is closely related to the diagnostics of changes in mineral and bone metabolism, especially in patients with kidney dinase. In these patients, especially those with chronic renal disease on dialysis, calcemia varies greatly along with the concentration and presence of individual fragments of PTH in the blood with cumulation of C- and non-(1-84) fragments, which include 95% of total PTH in the blood. There is currently a wide selection of methods for determination of PTH, which are both the second and the third generation methods. Therefore, the results of analyses are clinically difficult to compare. Accordingly, we decided to compare the stability of PTH depending on the method of collection, i.e. depending on the kind of primary material and storage conditions of primary samples before preparing their own analytical sample (the temperature and the storage period of primary samples). We attempted to assess the influence of these factors also with regard to the used determination method or its generation respectively. Our results confirm that the most preferred primary material for determination of the PTH concentration is blood collected into test tubes containing the K3EDTA additive. Especially during the expected or necessary longer material transport to the laboratory, the test tube with the K3EDTA additive appears to be the most optimal, since its PTH concentration does not significantly change even after 24 hours of storage at room temperature. Stable PTH concentrations are during the selection of the primary material obtained both by the second generation method (Intact PTH, Immulite 2000 XPi) and by the third generation method (PTH (1-84) biointact, cobas e411).
