The aim of the study was to describe trends in patients with multiple sclerosis (MS) initiating their first disease‑modifying therapies (DMTs) in the Czech Republic and to present the history, data structure, completeness, and scientific value of the national MS registry ReMuS. The number of MS patients recorded in ReMuS increased from 9,019 in 2013 to 17,478 in 2021. While the proportion of patients treated with any DMT ranged between 76% and 83%, the percentage of individuals initiating high‑efficacy DMTs (HE‑DMTs) rose substantially—from 2.1% in 2013 to 18.5% in 2021. A total of 8,491 treatment‑naive patients started DMTs during the study period. The study highlights the increasing use of HE‑DMTs, their therapeutic potential and associated risks, and emphasizes the crucial role of long‑term real‑world monitoring enabled by clinical registries.
