Original language description
This case report presents an 18-year-old female patient with recurrent calcium kidney stones and normocalcemia masking primary hyperparathyroidism. Initial laboratory evaluation revealed vitamin D deficiency, hypomagnesuria, mild hypercalciuria, and elevated parathyroid hormone (PTH), supporting a diagnosis of secondary hyperparathyroidism. Following vitamin D supplementation, persistent PTH elevation together with the development of hypercalcemia uncovered an underlying parathyroid adenoma, which was surgically removed, leading to normalization of biochemical parameters. The case emphasizes the essential role of 25-OH vitamin D measurement in the differential diagnosis of calcium nephrolithiasis and demonstrates that combined primary and secondary hyperparathyroidism may clinically present as normocalcemic stone disease.
