This research letter published in the Journal of the American Heart Association assessed the prevalence of patent foramen ovale (PFO), its likely causal role in embolic stroke of undetermined source (ESUS), and prognosis in young patients. The analysis used the multicenter prospective Y-ESUS cohort (13 countries; enrollment 2017–2019) including patients ≤50 years with recent ESUS, restricted to those with TEE-confirmed PFO and sufficient anatomic detail for PASCAL classification (probable/possible/unlikely PFO-related stroke). PFO was frequent in the registry (≈50%), and most cases were categorized as probable or possible by PASCAL (≈85%). Over a median follow-up of ~18 months, no recurrent ischemic strokes occurred among patients with PFO; PFO closure was performed in roughly one-third and appeared only weakly related to estimated PFO pathogenicity. Overall, the findings support a high prevalence of PFO in young ESUS and suggest a low short-term recurrence risk, while highlighting limitations due to variable echocardiography protocols and incomplete TEE ascertainment in part of the cohort.
